I used to have Internet Security and just upgraded to Total Security. I thought this would provide premium access to VPN but is it always purchased separate?

Sadly no, KTS includes free Kaspersky Secure Connection - VPN, the VPN is automatically installed when KTS is installed:

• KVPN standard (free) has certain limitations:

1. Protected traffic per day: 200 MB, (300 MB if the application is connected to My Kaspersky)
2. Remote virtual server, country selection - automatic. 
3. Maximum number of connected devices: Unlimited
4. May have advertising. 
5. No access to Kaspersky Technical Support.
